The Centre for Mental Health Addiction (CAMH) is a large, academic mental health facility in Toronto, Ontario, employing 426 physicians. They implemented Cerner Millennium® six years ago and have achieved HIMSS Stage 7 and Davies Awards for their use of data in improving health outcomes. The organization has been implementing a broader wellness strategy and the use of the EHR has been factored in. EHR usage is commonly attributed to physician burnout, and CAMH took deliberate steps using existing staff to address those concerns and engage with end users in a structured way. Join this session to learn how CAMH formed a SWAT team of 10 multidisciplinary staff and engaged with the physician chiefs across the organization to gather EHR requests. Using an agile approach to project management, the requests were assessed through monthly meetings with the divisions and measures implemented from training through to system changes.

The CareView Discharge Progression board is a multi-disciplinary patient throughput visual management system used to coordinate and sequence efficient and high-quality care planning for patients and families. The CareView huddle process supports proactive coordination and prioritization of patient-centered tasks to ensure discharge occurs at the right time and the right location. The huddles occur three times per day at the electronic board and promote a team focus on discharge goals to support overall throughput targets. Successful adoption of this kind of people, process and technology change requires a culture of innovation and strong collaboration between point of service leaders, IT and informatics.
Seamless Exchange focuses on interoperability capabilities that collect, curate and organize data from disparate systems across the health IT landscape to build a complete longitudinal record. The COVID-19 pandemic has focused global attention and resources on improving public health reporting systems. Cerner adopted the new FHIR application, eCR Now, in 2020 and began testing and deployment at LMH Health (LMH), which resulted in a go live in April 2021 and general availability in July 2021. This breakout session will review eCR Now and Cerner's eCR solution, discuss the Centers for Medicare and Medicaid Services (CMS) rule requiring eCR attestation for Medicare Promoting Interoperability Program incentive payments in 2022, and share LMH’s experience and outcomes using the app.

Encompass Health, the nation’s largest system of rehabilitation hospitals, has collaborated with Cerner to develop predictive models to identify patients at risk for an unplanned acute-care transfers, as well as falls risks, by leveraging original research, machine learning and statistical analysis. The goal is early identification of risk with early interventional strategies so the patient can complete the rehab experience successfully and return home as soon as possible. The acute-care transfer functionality will soon be available to Cerner clients.

Healthy Wirral Partners (HWP) works with the Lewisham area of London to support these vulnerable patients during the pandemic and beyond. In this session, hear how HWP and Lewisham used HealtheIntent and data science to identify patients with learning disabilities as a priority group for COVID-19 vaccination. They’ll also detail the other work being done in Lewisham to improve the health and wellbeing of this at-risk population.
