Enhancing structured medication reviews with digital tools

Healthy Wirral Partners (HWP) and clinical pharmacists from the local primary care networks collaborated with Cerner and Imperial College London to co-develop and implement digital tools that enhance structured medication reviews in primary healthcare settings.
